Members are entitled to one (1) dive tag upon joining the club. Additional tags may be purchased for $4 each.
Submit a separate form for each member applicant and associate member applicant(s).
Your tag will be processed upon membership confirmation and receipt of payment if ordering more than one (1).
- Complete all fields listed. Incomplete form will not be processed.
- Please type with all CAPITAL LETTERS. We are not responsible for type mistakes.
- Punctuation available: comma (,), apostrophe (‘), dash (-), C/O and 1/2.
- Payment for additional tags is due before processing. Make checks payable to Berkeley Surplus and submit to the club Secretary, at the General Meetings (first Thursday of every month, except December).
- All tags must be picked up at the General Meetings.
- Recommended information: name, in case of emergency (ICE), phone number, DAN id number, allergies, cell phone number.
